Will my insurance cover any treatment done at your facility?

Our receptionist will take all of your insurance information and check to see if it is a plan that we accept.   If so, this will cover your initial evaluation, but may not cover any other treatment you need.  Insurance companies may deny coverage for various procedures done at our facility.  Stem cells and other procedures have been used for decades but are relatively new to pain management.  If insurance does not cover your treatment with our facility, we are happy to offer payment options/plans.

What if you do not accept the type of insurance I have?

If we do not accept the type of insurance you have, we are happy to offer a payment option/plan that works for you.  Initial Consultations are $350 while follow-up appointments are typically $150.  

What is the cost for treatment?

The cost of treatment varies based on the service or procedures, consultations.

What will MY first visit consist of?

During your first visit, your provider will perform a complete and thorough physical examination. 

You will have ample time to discuss your initial diagnosis and all your questions will be answered in detail.  Additional testing may be required for further diagnosis.  

What do I need to bring to my first visit?

Please bring your insurance card and a form of photo identification to your first visit.  You will not be seen without a photo ID. Make sure to also bring with you any relevant medical records such MRIs, CT scans as well as laboratory results.  Anyone under the age of 18 must be accompanied by an adult for all visits.

What if I miss an appointment?

We ask you to confirm your appointment at least 24 hours in advance.  If an appointment is missed or is not cancelled 24 hours in advance, you may be charged $50 for a follow-up visit and $100 for the initial evaluation.
